International audience ; Treatment of Maghreb patients is characterised by the practice of medical pluralism. Many of these patients, immigrants or born in France, resort to practices popular in the Maghreb (fqih, roqya, prayer and dewitching). The existence of these practices can be explained as much by maintaining the culture of the country of immigration and the social trajectory as by the time spent in the host country or the migration experience. The patients and their relatives are not anchored in a culture of origin, which explains their actions as they do not believe in or fully adhere to the aetiology of mental disorders. An available solution, easy to access, and dissatisfaction of psychiatry explain the pragmatism demonstrated by patients and their relatives who use a variety of treatments. Popular remedies may be carried out in France or when returning to the country of origin where Maghreb patients may also consult psychiatrists with whom they share cultural sensitivity. Medical pluralism of Maghreb patients in France represents a transnational modality. ; Les itinéraires de soins des patients maghrébins adoptent souvent des formes de pluralisme médical. Ces patients, migrants ou nés en France, recourent en effet pour bon nombre à des soins traditionnels au Maghreb (fqih, roqya, prière, désensorceleur). L'existence de ces pratiques s'explique tout autant par le rapport entretenu avec la culture du pays d'émigration et la trajectoire sociale que par le temps passé dans le pays d'accueil ou l'expérience migratoire. Les patients et leurs proches ne sont pas ancrés dans une culture d'origine qui expliquerait leurs recours car ils n'impliquent pas la croyance ou une adhésion pleine et entière à une étiologie des troubles mentaux. Une offre disponible, facile d'accès ainsi que des insatisfactions vis-à-vis de la psychiatrie expliquent le pragmatisme dont font preuve les patients et leurs proches en recourant à une diversité de soins. Les recours traditionnels peuvent être réalisés en France ou lors d'un retour au pays d'origine, ici et là-bas. Mais lors de leurs retours au pays, les patients maghrébins peuvent également consulter des psychiatres dont ils apprécient la sensibilité culturelle. Le pluralisme médical des patients maghrébins en France représente une modalité de l'expérience transnationale. Il convient d'abord de noter que notre question, centrée sur le recours, exclut tout un ensemble de travaux, nombreux, sur la santé mentale des migrants ou sur les soins en santé mentale ailleurs qu'en France et sur d'autres continents. La question des recours aux soins des migrants a été abordée en France essentiellement sous deux angles. Le premier met l'accent sur le recours comme ensemble des possibilités d'être pris en charge dans le système institutionnel dominant au regard des conditions de vie précaires et du droit-et particulièrement du droit des étrangers, des politiques de santé et de l'immigration. Les travaux qui s'y inscrivent montrent les difficultés d'accéder aux soins, en particulier du fait de discriminations ou de droits spécifiques et restrictifs (Revue européenne des migrations internationales,). La dimension culturelle des pratiques s'en est trouvé ignorée. Le second angle porte sur ce qu'il convient de faire ou pas pour soigner les migrants. Il existe en effet une pluralité de théories et de pratiques professionnelles qui se proposent de les soigner en prenant en compte d'une manière ou d'une autre le bain culturel au sein duquel ils ont été socialisés (Baubet et Moro, 2003). En France, un courant de l'ethnopsychiatrie, développé autour de T. Nathan, a donné une tournure radicale a cette volonté en créant des dispositifs de soins inspirés des thérapies traditionnelles et en interprétant leurs souffrances et leurs pathologies à partir de la culture d'origine. International audience ; In this article we have chosen to focus on the circulations related to therapeutic mental health quests in the countries of the South through the example of Senegal. Mental health is not one of the priority health programs supported by the Global Fund, few countries in the South have specific mental health policies and programs, and it is not the subject of strong political and financial mobilization. According to WHO estimates, the majority of people with severe mental disorders are untreated and have diminished rights. With more accurate measurement of the disease burden, mental health has been included in the objectives of sustainable development and in overall public health. It is, in addition, a development issue given its social and economic costs to countries and families. Nevertheless, research on mental health in sub-Saharan Africa, whatever the discipline, is still too rare.Senegal was chosen for this research for two reasons: first, since French colonization, it has had an internationally recognized psychiatric range of services, and second Senegalese society is characterized by international migration and significant circular mobility, which anchors it in processes of globalization. The various circulations that contribute to structuring therapeutic pathways that are often long and chaotic before accessing psychiatric care are described mainly on the basis of anthropological data collected from people experiencing mental illness, either alone or supported by "their accompanying person(s)". These data are based on field surveys conducted in several psychiatric services and mental health centers throughout the country. Consultations were followed in agreement with the medical and administrative authorities, observations and interviews were carried out with healers, sick people and their "companions" in order to understand the therapeutic pathways, the management of the disease and drug treatments as well as the representations of psychological disorders in a context of stigmatization of madness. The investigation continued beyond the walls of the psychiatric institution with healers, marabouts and representatives of mystical medicine in order to take into account the plurality of therapeutic remedies and the movements that take place at local, national and even transnational levels. The interviews were conducted either in French or Wolof (translated by an investigator trained in health anthropology).The circulation of actors, products and practices is analyzed at different spatial and social scales and interpreted in relation to different explanatory and contextual factors. In the Senegalese health care system, mental health is not included in the primary care system. Map 1 reveals an insufficient supply of psychiatric care and deep inequalities between the West (the capital Dakar region) and the East of the country, as with other medical disciplines that are under-staffed and under-resourced. Sick people who consult in health centers or dispensaries are referred to the psychiatric unit in the health region where they reside or to the private sector if they have sufficient resources. Nevertheless, the pyramidal medical hierarchy is not always respected because it goes against the social logic of patients and their families who wish to consult in the greatest secrecy because of the social representations of madness. Therapeutic pathways therefore depend in part on accessibility and the additional costs of distance.The accounts of the therapeutic pathways reveal that psychiatry is often considered the ultimate solution in the event of an acute crisis, particularly in rural areas. It is a solution sometimes forced by the neighborhood or the authorities in the face of symptoms that cause fear and disapproval. Given the context of stigmatization and reluctance towards psychiatric institutions, families are experimenting with other therapeutic alternatives with healers and marabouts who may specialize in the treatment of madness or in a so-called "mystical" medicine that is rooted in Islam. This is also due to certain popular beliefs that insanity is not curable, which hinders adherence to psychiatric treatment. This leads to the interruption of treatment as soon as the patient is perceived to be doing better. The narratives of the interviewees also reveal self-medication practices that aim to regulate the behavior of sick people in the family and social space of the community. It helps to limit medical and indirect costs related to accessibility, as many families do not have universal medical coverage or sufficient financial resources. It reveals the circulation and sometimes diverted use of medicines after a medical check-up. In conclusion, we observe that the question of the management of psychological disorders is similar to that of chronic diseases (adherence to treatment, representation of recovery). It is also part of a context where a plurality of actors are taking hold of a protean "psy" discourse within Senegalese society. ; A partir de données anthropologiques et sociodémographiques collectées au Sénégal auprès de différentes catégories d'acteurs impliquées dans les quêtes thérapeutiques qu'engagent des personnes malades, seules ou soutenues par « leur(s) accompagnant(s) » afin de remédier à une pathologie mentale, sont décrites diverses circulations qui contribuent à structurer des parcours thérapeutiques souvent longs et chaotiques avant d'accéder à des soins psychiatriques. Les circulations des acteurs, des produits et des pratiques sont analysées à différentes échelles et interprétées par rapport à différents facteurs : l'inégale répartition territoriale de l'offre psychiatrique, les représentations des troubles psychiques et de leur guérison qui les rapprochent des maladies chroniques dans un contexte de pluralisme thérapeutique et de stigmatisation, certaines pratiques d'automédication au sein des familles en réponse au coût des soins en l'absence de couverture médicale universelle généralisée, enfin l'émergence de discours « psy » au sein de la société sénégalaise.

Résumé Au cours de la première moitié du xix e siècle, les Vietnamiens ont été les instigateurs d'un recours grandissant aux médicaments occidentaux et en particulier aux produits pharmaceutiques européens. Cette augmentation de la consommation médicamenteuse dans le Viêt nam colonial ne semble pas avoir forcément signifié une confiance accrue dans le médecin occidental ni un rejet des médecines et des remèdes dits traditionnels. Nous nous penchons en conséquence sur l'articulation entre les phénomènes d'automédication et de pluralisme thérapeutique, les logiques qui les sous-tendent, et la reconfiguration de l'offre et de la demande en médicaments dans le Viêt Nam de l'entre-deux-guerres.

By investigating these issues, this paper aims to shed light on some of the complex ways in which practitioners and patients who take part in a local method of healing, in this case ritual healing through possession, respond to the expansion of biomedicine.The first part of the study analyzes the local landscape of medical pluralism, with particular attention to the local ritual system, which has been for a long time the dominant healing technique and which serves as a political regime for the control of bodies. The second part sets out the therapeutic itineraries and experiences of the actors engaged in these journeys. Following the WHO recommendations made in the late 1970s, many countries in Africa have implemented dynamics of recognition of traditional medicine and traditional healers. This dissertation focuses on the development of « integrative medicine » in Madagascar, consisting in its integration into the formal and conventional health care system. This development project, focusing on improving population's public health, requires a double evaluation: a social evaluation with the supervision of traditional healers' practices and a therapeutic evaluation with research on medicinal plants and development of improved traditional remedies. This research is the result of a methodological approach based on multi-located fieldworks conducted in the Malagasy capital and its surroundings and an ethnography of a rural town in the Analanjirofo region (east coast of Madagascar). On one side, this approach focuses on the attempts of application of political decisions regarding the supervision of traditional medicine and on the other side, on the real practice of traditional care as well as on individual and communal behaviours when facing disease, sickness, illness and misfortunes with extended case studies. This thesis contributes to the anthropology of health with a focus on political and developmental dynamics. A special emphasis is placed on the nature of cohabitation existing between practitioners (doctors and traditional healers) going from a simple referencing of patients to a rare collaboration.
